"Chronic Care Management and Traditional Approaches Workshop"

Chiefs of Ontario is pleased to announce that we are coordinating a workshop for Community Support Services and Home and Community Care. The workshop is titled “Chronic Care Management and Traditional Approaches”. The workshop will be held at the Courtyard Marriott, 475 Yonge Street in Toronto on January 22nd and 23rd, 2008.

The workshop was designed to help workers gain more knowledge surrounding culture and tradition and how incorporating these aspects can assist them within their roles. Some of the topics being discussed include Traditional Approaches to Healing and Wellness (as related to Chronic Disease Management), Cultural Adaptation to Diseases in Everyday Living as well as success stories from other First Nations members. An agenda will be available at a later date for a full list of topics.

The workshop is being sponsored for ONE person within Community Support Services (CSS) and ONE person within Home and Community Care (HCC) per First Nation. If your community is running only one of these programs, we can only sponsor one person due to limited space (i.e. You have CSS but not HCC, you cannot send two people from CSS).

Chiefs of Ontario will direct bill your accommodations (once a participant registers we will add that name to the hotel list) and will reimburse for travel, incidentals and meal expenses at Chiefs of Ontario rates. All travel arrangements (i.e. booking air travel) will be at the responsibility of each individual. Travel expense claim forms will be available at the workshop so please remember to retain all receipts for remittance.
