Aboriginal Youth Basketball Clinic November 12, 13, 14
The aboriginal Youth Basketball Clinic is happening on November 12, 13, 14 2010 in Thunder Bay, ON. This event is for males born between 1993 and 1998. Along with Aboriginal Team Ontario coaches and volunteers; coaches and players from the Lakehead University Thunderwolves are going to share their skills and talents with the participants. The clinic is being held at Pope John Paul II School, 205 S Franklin st and at Dennis Franklin Cromarty High School, 315 N Edward st. There is no cost to register, but space is limited, so get your registration in early! Registration closes NOVEMBER 5, 2010.
Attached you will find the registration package. There are 12 pages including an introductory letter and a poster of the event. Completing the forms not only protects you but it also provides the information and permission for the organizations to provide a safe and inclusive environment. The following forms MUST be completed:
Participant code of conduct
Universal Consent Client disclosure form: for the collection use and disclosure of personal health information
Release of liability, waiver of claims and indemnity agreement
Programme Registration form
Par-Q & You, only complete if you are 15 years of age or older
Photo release form
